The State of California requires these forms
to be on file in the WCCS office before
your student may start school.
** NOTE: Any PDF documents marked with two (2) asterisks can be typed into from your computer using Adobe Acrobat Reader. Please note, however, that in most cases these docs cannot be saved, so you will want to print as soon as you are done typing. Also note that signature lines must be signed by hand.
